In Coaching Analytics & Insights, forms data is now available within the reporting engine. This data can now be filtered, graphed, and analyzed to pull very rich data. Since the survey data (and the types of questions) can come in many forms, there is not a standard report that could accomplish all of the variations. Instead, custom dashboards can be built to showcase the response data in limitless ways. It is very easy to create brilliant dashboards based on response.

There are many options to setting up forms/surveys in terms of the types of questions (and responses). The form build allows for you to add different questions to the form, and may include the following question types:

There is also a feature called answer weight, which may be very important when it comes to reporting. The answer weight (shown below) allows you to provide numerical values for responses that may be in text. By adding an answer weight to your forms, this data can then be used in calculations, like averages, in reporting.

The Form Submissions datasource has all of the responses that have been collected from external participants of from within engagements. It includes information about the engagement, coach, coachee, along with all of the form related information such as the name of the form, the question text, the answer, and the answer weight (if applicable).

The screenshot below shows the view when building a custom visualization. First create a Custom Dashboard. Then click "+ Visualization". Choose the Form Submissions datasource, and then you will see the following.

From here you can choose the visualization type, select the metrics to measure, and also setup filters for the data. You can be very creative and build robust charts that showcase the data. Below is a segment of the Engagements Dashboard, that is built off of the same Form Submissions data source.
